One more step before camp.

Every family or adult attending Occupied needs to complete the Event Registration and Medical Information form (Form E6) by 17 December - one week before camp - so our leaders know about allergies, medical conditions and who to call in an emergency. It takes about five minutes.

Why we ask

Safe Places, safe camp.

Form E6 is the Seventh Day Adventist Reform Movement's standard ChurchSafe and Safe Places form for events. The details you give go only to the camp leadership team and are used to keep you and your children safe during the ten days at Elim Heights.

Already registered?

Good - this form is separate from your camp registration. If you registered before this form was available, please complete it now. One form covers every adult and child in your registration.

Serious allergies?

Bring your allergy action plan and prescribed medication to camp and tell a leader where they are kept. In cases of serious allergies, meals may not be able to be provided at every event.

Questions? Contact us at youth@sdarm.org.au.

Your signature is captured electronically. A copy of the completed form is kept by the AUC Youth Department; you will receive a confirmation email once it is submitted.
